Aims and auditable outcomes: We aim to ensure 100% patients on clozapine have annual physical health checks. By ensuring all prescribed therapy receive an check medic review, we improve patient safety prevent serious harm from occurring in cases that could be avoided. Methods All aged over 18 years Clozapine, who were under the assessment treatment service Eastbourne, identified using Carenotes, our electronic records system. Results 78% of had been reviewed by a doctor past 12 months. 32% attended review within One not medical for several years. Conclusion Our audit has shown there are no clear guidelines long term monitoring regards reviews psychiatric assessment. Using best practice it appears should minimal standard, however further evaluation this is recommended at trust level.
